DuPont Sustainable Solutions has released a new four-part training series on incident investigations. Available on DVD and as online courses, the Incident Investigation series is designed to help organizations hone their analytical and practical skills to improve the quality of safety investigations and better manage workplace risks.

Available on DVD and as online courses, the Incident Investigation series is designed to help organizations hone their analytical and practical skills to improve the quality of safety investigations and better manage workplace risks.

The series follows a group of employees as they conduct an investigation that is presented in four parts as follows:

Incident Investigation: Getting Started defines what an incident investigation is, explains the importance of the process, identifies key roles and responsibilities, and gives an overview of each procedure in the process.


Incident Investigation: Forming an Effective Team and Gathering Information discusses the composition of an incident investigation team, a three-pronged analytical framework that governs the fact-finding process, and interview techniques and practical communication strategies.


Incident Investigation: Analyzing the Facts and Causes shows how to consolidate the data, how to analyze the information and determine key contributing factors through a root cause analysis, and how to link key factors to specific management systems that affect how a task is performed.


Incident Investigation: Recommendations, Communication and Follow-Up, shows how key contributing factors have to be corrected or prevented, explains how lessons learned, priorities, and expectations must be disseminated throughout the organization, and lists follow-up procedures from creating action plans to monitoring procedures.
